Treatment of Lumbar Disease with Posterior Lumbar Interbody Fusion



Abstract
Posterior lumbar interbody fusion(PLIF) was performed on 22 patients on 22 patients who had lower lumbar disease. Eleven patients had spinal stenosis and/or spondylolisthesis, 7 had internal disc disruption, 2 had failed back surgery syndrome and
the
remaining two were one spinal fracture and one segmental instability. PLIF alone was done in 7 patients and PLIF was augumented by pedicle screw fixation in 15. There were 15 males and 7 females and mean follow-up and 23.5 months. Sixteen
patients
had
pathology in L4-5 and 7 had in L5-Sl.
Authors analysed the clinical and radiologic results of the whole patients and compared the result of instrumented and non-instrumented groups. Operation time was longer in the instrumented group. Pain score assessed by patient's subjective
visual
numeric scale was improved in both groups. Satisfactory results were obtained in 16 out of 22 patients(73%).
Radiologic stable interbody fusion was observed in 20 patients(91%). Two patients in the non-instrumented, group showed nonunion. Every patients in the instrumented group showed solid fusion. One patient in the instrumonted group developed
neurologic
deficit after operation.
In conclusion, PLIF is a good method to achieve spinal fusion when it is augumented by spinal instrumentation. To avoid complications, careful and precise surgical technique is necessary.
